Instructions for filling the OMR Application Form for IBSAT® 2008

Please read the guidelines carefully before filling the OMR Application Form for the IBS Aptitude Test (IBSAT® 2008).

General Guidelines:

The application form will be scanned by Optical Mark Reader (OMR), a computer-based machine. Therefore, utmost care should be taken while filling the data in the application form.

arrow Mark your response only in the space provided for the purpose. Response marked elsewhere will not be considered. Fill the boxes in the form first and then proceed to darken the relevant circles.
arrow Use HB Pencil only for filling up information items I to 14 of the OMR application form. Please darken the appropriate circles with due care. Avoid over-writing.
arrow Completely darken the respective circle for your response. You may change a mark by completely erasing the wrongly filled circle. The erasure should be complete and without any smudges as partially erased mark may lead to wrong reading by the computer.
arrow Fill up the OMR Application Form using black / blue ball point pen from item no.15 onwards.
arrow Please do not write or mark on this application form outside the demarcated areas.
arrow Do not pin or staple any thing with the OMR application form.

Staple the attested copies of marks sheets of X, XII and graduation together in the ascending order that is X on the top and graduation at the bottom.
arrow Insert the duly filled in OMR application form and the marksheets in the pre-addresed special envelope provided and dispatch/deposit envelope to:

arrow The OMR application form should not be folded.
arrow Do not make any stray marks on the OMR application form.
arrow Keep a photocopy of the completed OMR application form for your records.
arrow Ensure complete correctness of the details provided in OMR application form.
arrow While stating the year under appropriate items, (eg: date of birth or year of completing qualification), use the notation as given in the following examples:
arrow Year 1998 should be indicated as

arrow Year 2008 should be indicated as
0 8

Guidelines for filling individual items

The OMR application form contains 22 individual items. All items have to be filled in carefully, as per the guidelines, before dispatching the completed application form.
1. Name of the Applicant: Write your name, in capital letters, in the boxes provided. Leave one box vacant between each initial. There is provision for 35 boxes only. Use the boxes, including vacant boxes, carefully.
  Example:

Full name: Anand Kumar Khanna

Write as:

If your name exceeds 35 characters then you may write as:

Darken the corresponding circles accordingly.
2. Application Number: The OMR application form has a unique 6 digit application number. Write your application number in the boxes and darken the corresponding circles.
3. Gender : Darken the appropriate circle.
4. Citizenship: If you are a citizen of India, darken the circle for ‘yes’, otherwise darken the circle for 'no'
5. Test Center : You can choose any one test center available for IBSAT® 2008. Enter the test center code number in the box and darken the corresponding circles. In the event of not having enough seats at the test center requested by you, you will be allotted the nearest alternative test center.
Codes for Test Centers are given below: Please note the following code numbers of the test centers given in the brackets. Enter the code in the box and darken the corresponding circles under item No.5. As this is very important, please double check the code.

  Mode of Study: Darken the appropriate circle to indicate whether you have acquired the qualification through the full-time mode or part-time mode or through distance learning.
  Duration of Program: Indicate the duration of program in terms of number of years by darkening the appropriate circle.
  Status : Indicate whether you have completed your bachelor degree program or currently studying the final year by darkening the appropriate circle
Aggregate Marks:
 
  If completed: If you have completed your bachelor degree program, enter the percentage of aggregate marks in the box and darken the appropriate circles.
  If studying: If you are in the final year of your bachelor degree program, enter the percentage of aggregate marks obtained for the years/semesters of the bachelor degree program completed till date and darken the appropriate circles.
 
  Example:
(A) A percentage of 81.01 to 81.49 should be indicated as

The decimals from 0.5 and above have to be rounded of to the next higher number
  Year of Passing: Indicate the year in which you have passed the final year of the degree examination. If you are in final year of bachelor degree program indicate the likely year of completion of the bachelor degree program.

Aggregate Marks(%): Indicate the aggregate percentage of marks obtained by you in the PG Program. Applicants currently in the final year of their PG Program should give the aggregate percentage up to the current semester/year.

  Year of Passing: Indicate the year of passing of the final year of the
PG Program. If you are in the final year of PG Program indicate the likely year of completion of PG Program.
(e) Professional Qualification:
  Qualification: Codes for professional qualifications are given below. Select appropriate code and darken the corresponding circles.

10. Work Experience as on May 01, 2009: If you do not have any work experience, darken the circle corresponding to ‘nil’. If you have work experience, indicate work experience in years in the corresponding circles. The work experience after the graduation is only to be indicated.
11. Address: Write your mailing address clearly in the boxes and darken the corresponding circles. Do not write your name.
12. State and Union Territory Code: Select the state/UT code correctly for purpose of your mailing address from the given list to indicate your State/Union Territory.

13. Pincode: State the pincode of your city/ town as a part of your mailing address accurately in the boxes and darken the corresponding circles.
14. Mobile Number: Indicate your mobile number in the boxes provided to you.
15. Name: Write your name in capital letters the way it is mentioned in your graduation certificate.
16. Telephone Number: State your contact telephone number. Indicate the STD code of your city/town/village followed by the telephone number. Do not leave a blank space in between the STD code and the telephone number.
17. E-Mail Address: E-mail will be extensively used during the admission process by IBS Admissions Department to intimate the applicants of the important developments. Please ensure that you fill it carefully, legibly and accurately.
18. Name & Address of the College: Write the complete name & address of the college from which you have completed/pursuing your graduation.
19. Name of the University: Indicate the name of the University from which you have completed/pursuing your graduation.
20. Indicate complete details of executive level work experience after graduation in descending order starting from the current employment including Employer’s Name and Address; Period of Service; Designation and Area of Work. No need to enclose any certificates along with OMR Application Form.
21. Family Income: Please tick the appropriate box which indicates annual income of your family.
22. Declaration: Please read the declaration carefully and sign.

Color Photograph: Affix your recent color photograph (Passport Size).
